Cleveland Clinic Social-Worker-Hospice-part-time in Independence, Ohio

Job Summary:

Responsible for completing a thorough assessment of social services need of patients and families. Develops and implements plan of care based on assessment and observational skills.


  • Master's Degree in Social Work from an accredited school of Social Work. (CSWE)


  • At least 2 years clinical experience as a Licensed Social Worker.


  • Current licensure in the State of Ohio as a LSW or LISW.

  • Basic Life Support (BLS) certification required

  • Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.

  • Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.

Required Physical and Environmental Demands

  • Ability to sit/stand for 85% of the time.

  • Must be able to climb stairs, pull, push and facilitate movement of the patient.

  • Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds.

  • Must have correctable vision.

  • Must have correctable hearing to be able to hear normal conversations.

  • Must present professional appearance at all times.

  • Must have reliable means of transportation


  • Performs and documents a Social Work assessment within 3 working days of initial referral, utilizing interview, observation and other available information. Establishes a written patient plan of care on primary patients and documents intervention within 72 hours.

  • Visits/contact each patient and family according to established plan of care. Implements the established plan of care with each visit. Evaluates the patient and family and provides appropriate intervention with each visit. Revises/updates patient care plan at each visit/contact based on change in patient condition/status or response and evaluation/effectiveness of interventions. Documents plan of care revisions every 2 weeks on the Interdisciplinary Team plan of care review.

  • Counsels patients/families/caregivers as needed in relationship to stress, and other identified coping issues. Demonstrates appropriate psychosocial intervention based on assessment data when working with specific high risk patient populations. Assesses and documents the patients/families level of knowledge related to the patient's specific needs with each visit. Interprets the hospice mission, philosophy and policies to patients and families on initial visit and as needed.

  • Evaluates and documents the effectiveness of the education provided with each visit. Uses verbal and written communication to ensure continuity of care by participating in Interdisciplinary Team plan of care revision and completing appropriate documentation.

  • Provides information and acts as a liaison for patients/families regarding community resources to assist with identified needs. Maintains comprehensive listing of available community resources. Facilitates/assists with referrals to community agencies when requested to do so to meet patient/family needs. Sets/revises priorities for one's daily assignment based on patient needs/acuity, patient's family request and available resources.

  • Participates in departmental committees/projects as appropriate. Participates in bereavement program through attendance at memorial service.

  • Performs other duties as may be assigned.
