Cleveland Clinic is currently seeking experienced Family Nurse Practitioners for an exciting new on-demand urgent care telemedicine service called Express Care® Online. Other aspects of the position include management of other digital care delivery technologies. The successful applicant will treat patients ages 2 and up online through the use of live, audio-visual technology for a variety of minor medical conditions such as: cough and cold symptoms, yeast infections, sinus infections, seasonal allergies, earaches, skin rashes, bronchitis and more. The successful applicant's home base will be out of the Cleveland Clinic Administrative Campus, located in Beachwood, Ohio. Hours are variable and weekend availability is preferred. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Provides advanced nursing care to patients with complex acute, critical, and chronic health conditions, including the delivery of acute care services. Practices with a variety of specialty-based populations including neonatal, pediatric, and adult. Practices in any setting in which patients require complex monitoring and therapies, high intensity nursing intervention, or continuous nursing vigilance within the range of high-acuity care.
Assesses health status by obtaining relevant health and medical history.
Performs a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician.
Identifies significant health and medical risk factors.
Documents findings in medical record.
Orders, interprets and evaluates laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs.
Develops a diagnosis by analyzing and synthesizing collected data.
In collaboration with the physician, develops a treatment plan along with the patient and significant others.
Performs diagnostic or therapeutic interventions consistent with certification, law and Cleveland Clinic privileges.
Prescribes and monitors medications as allowed by prescriptive authority through applicable state Board of Nursing and CCF policy.
Develops patient education plan. Participates in medical team rounds.
Collaborates with nursing, medical and other healthcare team members regularly to ensure quality, safe patient care.
Performs admission history and physical; documents progress notes; prepares discharge summaries as required.
Assists with and facilitates discharge planning.
Other duties as assigned.
- Effective 05/01/2014: MSN required upon hire or within 5 years of hire for external hires and internal hires moving from a non-APN licensed role to an APN licensed role.
Licensed as Registered Nurse (RN) in applicable state, national certification as nurse practitioner (APN) by nationally accredited organization, current certificate of authority by applicable state Board of Nursing, certificate to prescribe (CTP) preferred.
Basic Life Support (BLS) Healthcare Provider required; Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) may be required based upon work setting.
Complexity of Work:
Demonstrated mastery of nursing concepts and principles and ability to identify and use appropriate concepts and principles from related sciences; e.g. behavioral sciences and biological sciences.
Excellent analytical and decision making skills are required.
Strong written and verbal communication skills.
Ability to work as a member of a multi disciplinary team.
Demonstrated competency and the ability to perform advanced assessment, diagnosis, treatment plan development and invasive procedures as per clinical privileges.
- Two years clinical nursing experience preferred.
Typical physical demands include the ability to walk and stand for long periods of time.
Manual and finger dexterity and eye/hand coordination to perform physical examinations.
Requires corrected vision and hearing to a normal range.
May require exposure to communicable diseases and/or body fluids.
Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Medium Work.
